Assisted Living Program Application
Assisted Living Program Announcement: ALP 6000-Bed Initiative, SOI #2
Chapter 58 of the Laws of 2009 amended Section 461-l of the Social Services Law (SSL) to authorize the Commissioner of Health to establish up to 6,000 new Assisted Living Program (ALP) beds. This amendment requires that an equal number of Nursing Home beds be decertified for all new ALP beds authorized.
Interested applicants must submit the Solicitation of Interest form. This form and instructions as well as other relevant information can be accessed below:

Note: It is incumbent upon the interested party to thoroughly review their Solicitation of Interest request for completeness and accuracy prior to submission based on the instructions provided. The Department will select applicants based primarily on information provided in the Solicitation of Interest. Solicitation of Interest forms received for new ALP beds will be given preference over Solicitation of Interest forms received for the conversion of existing licensed ACF beds.
The Department will accept written questions received on the Department web site. All questions should be submitted via email, no later than October 6, 2010, with the subject line ALP 6000, to ALPapplication@health.state.ny.us. The Questions and Answers (Q&A) document will be posted on October 21, 2010.
